This application is the first step in determining your eligibility for admission and gathering the information required for enrollment, state licensure, and participation in our partnership programs with Lamar State College Orange.

To complete this application, please have the following information and documents available:

  • Government-issued Driver License or State Identification Card (if applicable)

  • Social Security Number

  • A recent passport-style or school photo (head and shoulders)

  • Information about your educational history, including high school, GED, college, and any previous career or technical education

  • Information regarding any previous cosmetology, barber, esthetics, manicuring, or eyelash extension training, including schools attended and hours completed

  • Information regarding any professional licenses you currently hold or have previously held

  • Information about any criminal convictions or disciplinary actions that may require review under Texas Department of Licensing and Regulation (TDLR) requirements. A criminal history does not automatically prevent admission or licensure, but additional review may be necessary.

  • Emergency contact information

  • Parent or guardian information (if you are under 18 years of age)
